Requirements for Membership form:

Effective Friday 1st December 2006, the following MUST be provided with all applications forms.

1. One (1) passport size photograph.
2. Two (2) photo ID’s, (National ID, Passport, Driver’s Permit)
3. Two utility bills. (T&TEC, TSTT, WASA)
   NB: In cases where the utility bill is not in your name a letter MUST be submitted by the property owner confirming address. In the absence of any utility bills the following will be accepted. (U.T.C. Statement, Bank Statement, Cable Bill, directly)
4. The application MUST be signed/recommended by an ACTIVE Member of the Society.
5. Please note the Personal Referee and the Recommender are two (2) different persons.
   EUSURE ALL FIELDS ARE COMPLETED. IF NOT APPLICABLE, PLEASE DASH (-) OUT FIELD/S.
